Understanding Acne Scars

Acne scars are textural changes left in the skin after inflamed acne has settled. When the inflammation reaches the deeper layers of the skin, the natural repair process can leave behind a change in the surface, most often a dent or depression in the skin. For many clients, atrophic acne scars, the indented kind, are the primary concern.

Acne scars are not the same as acne marks. Acne marks, the flat brown or red patches left behind after a spot, are pigment-led, not textural. They tend to fade over months on their own or with the support of pigmentation work.

Meanwhile, scars do not fade in the same way, because the change is in the structure of the skin, not just its colour.

Types of Acne Scars

Ice pick scars

Ice pick scars:
Narrow, deep, pinpoint indents that look as though the skin has been pierced

Boxcar scars

Boxcar scars:
Wider, more box-shaped depressions with relatively sharp edges

Rolling scars

Rolling scars:
Broader, gentler depressions that give the skin a wavy or undulating surface

Hypertrophic and keloid scars

Hypertrophic and keloid scars:
Raised rather than indented, more common on the jawline, chest, and back than the face

Acne marks (PIH):

Acne marks (PIH):
Flat darker patches left behind after acne, pigment-led rather than textural

Different scar types respond differently to different approaches, which is why the assessment matters before any plan is built.

Acne Scar Treatment at Journey Aesthetics

Most acne scar plans at Journey combine more than one approach. The combination is built around the scar types present, the surrounding skin, and what you can realistically commit to over time.

01 • Laser Treatment

Lasers used in acne scar work deliver controlled energy into the skin to support the body's own remodelling response. Different lasers reach different depths and suit different scar profiles.

Pico laser is often considered for surface texture and post-acne marks, where short pulses break pigment and prompt mild renewal with relatively short downtime. Meanwhile, Fotona laser works deeper into the tissue and is often considered for more pronounced atrophic scarring, where the response tends to develop more gradually.

02 • Chemical Peels

Chemical peels use a topical solution to support controlled exfoliation of the upper layers of the skin. The strength of the peel is matched to the case. In acne scar plans, peels are often used alongside laser work to support surface renewal and a more even skin tone, particularly where post-acne marks sit alongside the textural scarring.

For narrower, deeper scars like ice pick scars, a more targeted peel technique known as TCA CROSS (a high-strength solution applied directly into the base of individual scars) is sometimes considered as part of the wider plan.

03 • Topical Support

Topical support is the part of the plan that continues between sessions. Formulations recommended at consultation can support skin renewal and help maintain whatever the in-clinic work has done. The choice depends on your skin type, your current routine and what your skin is responding to.

Why Acne Scar Treatment
Must Be Tailored to Your Skin

Acne scars are rarely a single problem. Most clients present with more than one scar type, often alongside active or recent breakouts, and frequently with post-inflammatory marks layered on top. A standard protocol applied to that mix tends to under-deliver on some areas and irritate others.

A tailored plan also depends on life stage and skin context. For example, for a teenage client whose acne is still active, scarring work is generally not the priority until the underlying acne has settled. Aggressive laser work on actively inflamed skin can make things worse, and new scarring can continue to form while breakouts are ongoing.

Meanwhile, for an adult whose acne has settled but whose scars remain, the plan may combine deeper laser work with peels, topical maintenance, and sometimes targeted approaches, such as polynucleotide injections or subcision, for stubborn depressed scars.

At Journey Aesthetics, every acne scar plan starts with an assessment of the actual mix on your skin. Dr Chia will talk through which approaches make sense, in what order, and over what kind of timeline. The goal is gradual improvement built around what your skin will tolerate well.

Professional Background

Dr Chia Min Shan is the founder and medical director of Journey Aesthetics,
a personalised aesthetic clinic in Singapore. Her approach combines clinical precision with calm, considered care, with a focus on guiding clients through decisions rather than selling services.

Dr Chia's clinical focus is skin quality, facial structure and graceful ageing.
The services she works with regularly span injectables and energy-based devices, chosen and sequenced for each client rather than applied from a template.

Approach to Acne Scar Treatment

Dr Chia views acne scars as a category that rewards combination work and patience. Rather than applying a single approach across every case, she assesses each client's scar mix, surrounding skin, and broader context before recommending which combination of laser, peel, topical, and other approaches suits, and in what sequence. The goal is gradual improvement built around what your skin will tolerate well across multiple sessions.

Your visit at Journey Aesthetics.

Before

At consultation, Dr Chia will look at your skin under good lighting, ask about your acne history, current routine, and what you have already tried. The assessment shapes which combination of approaches is recommended, in what order, and over how many sessions. If a different route makes more sense than the one you came in expecting, she will advise you on your options.

Before Visit

During

Each in-clinic session varies depending on the approach used. Topical numbing cream is applied beforehand to reduce discomfort, and protective eyewear is worn during laser work. Most sessions take 30 to 60 minutes. The sensation varies by approach, often described as warmth, mild stinging, or a series of small snaps against the skin.

During Visit

After

Mild redness, warmth or tightness in the treated area is common in the first few days. Specific downtime depends on the approach used and how deep the work goes. Effects tend to develop gradually as the skin remodels over the following weeks. A course of sessions is usually recommended for cumulative support.

    Questions, Considered

    Things patients often wonder.

Is there downtime after acne scar laser treatment?

Downtime depends on the laser and the depth of the work done. Pico laser sessions typically involve mild redness or warmth for a few hours, with most clients returning to normal activities the same day.

Fotona laser sessions can involve redness, mild swelling, and a sandpaper-like texture for several days as the skin renews. Specific aftercare guidance is given at the appointment based on what was actually done.

How many sessions are needed for acne scar treatment?

Most cases require a series of sessions. The exact number depends on the scar types present, how deep the scarring sits, and which approaches are combined in your plan. A typical course often spans several months, with sessions spaced to allow the skin to respond between visits. The plan is built around your specific case at consultation.

Can acne scars be completely removed?

Most acne scars cannot be removed entirely, but their appearance can often be improved meaningfully with the right combination of approaches over time. The realistic goal is gradual improvement in skin texture and overall evenness, not a perfectly flat surface. The degree of improvement depends on the scar types present, the surrounding skin, and how consistently the plan is followed.

Does Journey Aesthetics offer personalised acne scar treatment plans?

Yes. As a doctor-led acne scar laser treatment clinic in Singapore, Journey Aesthetics builds each plan around the actual scar mix and skin type rather than a standard protocol. Combinations of laser, peel, topical support, and other approaches are sequenced based on what your skin is responding to. Restraint and observation lead the work, and the plan is adjusted as the skin changes.

How is acne scar treatment at Journey Aesthetics personalised to my skin?

Every acne scar plan at Journey Aesthetics starts with an assessment by Dr Chia, who looks at your scar mix, skin type, current acne activity, and previous skincare. The work is then shaped around what your skin is actually doing rather than a fixed package.

Is acne scar laser treatment suitable for adult skin?

Yes. Acne scar laser work is often considered for adult clients whose acne has settled but whose scars remain. Adult skin sometimes responds differently from teenage skin and benefits from a more conservative pace, with attention to skin quality alongside scar appearance. The assessment confirms whether a laser is the right route in your particular case.
